This research work is on critical examination of health record collection, storage and retrieval practices of health record staff in Abia State. In order to achieve this work, three research questions were formulated. The methodology used was descriptive survey design. The population was made of health record staff. Sampling size of 92 using a random sampling technique was adopted. The major instrument used for data collection was self-structured questionnaire which had two sections, section A has for personal data and section B focused on the different aspects of the problem under study. Frequency distribution table with percentage were used to analyze the data.
