DOCUMENTATION OF PATIENTS HEALTH RECORDS AND ITS IMPACT IN HEALTH CARE

This research work was on documentation of patients’ health Records and Its Impact in Health Care Delivery in Nigeria.

Having observed that most hospitals in the country do not ensure accurate documentation, this may be as a result of lack of trained personnel; also the associated problems are misplacement and loss of patient care information, high cost  of materials, inadequate power supply, space, delay in retrieval and storage, inadequate data for statistical reporting among others.
It is on this premise that this research was undertaken to find out or deal extensively with the issues of documentation of health care delivery and the objectives of this report are to identify the problems associated with documentation in health care delivery system,
