This research work is concerned with the challenges associated with disease coding and indexing among health information managers using Eleme General Hospital as a case study. It gives a general review of what disease coding and indexing is all about in that it highlighted the history of international classification of diseases ICD-10 and ethical standard of coding and indexing. The study observed that the basic challenges associated with diseases coding and indexing system are lack of personnel, inadequate facilities, poor wages and lack of in-house training which are some of the factors militating against effective disease coding and indexing system. The methodology used was descriptive survey design, with a population sample of 175 respondents. A random sampling technique was used to give every population equal chance of being represented.
