EFFECTIVE NUMBERING SYSTEM A REMEDY TO MISFILING OF PATIENTS CASE NOTES IN HEALTH CARE DELIVERY SYSTEM

This study was on Effective Numbering System, A Remedy to Misfiling of Patients Case Notes in Health Care Delivery System, using University of Nigeria Teaching Hospital (UNTH) Ituku/Ozalla, Enugu. In carrying out the study, the following objectives were stated; to find out the causes of misfiling of patients case note; to examine, if the misfiling of patients case notes is caused by the staff of the department. Based on the objectives four (4) research questions were formulated, where two hypotheses was postulated. A descriptive survey method was used in the research design. The populations of the research study consisted of senior and junior staff of the department of Health Information Management UNTH, Enugu that formed the targeted population of 170 with a sample size of 119. The instrument used for the research was questionnaire which was subjected to validity and reliability test.
