Electronic Health Record is a digital version of a paper-chart that contains all of a patient’s medical history from more than one practice. The fact that paper-based health records are rapidly becoming outdated, they are easily lost, are subject to tear, costly to handle, cannot be transferred electronically and may be difficult to interpret and inefficient. These fundamental drawbacks are driving a transition across the globe towards Electronic Health Record. Nnamdi Azikiwe University Teaching Hospital, Nnewi is still facing poor management of patients’ records as a result of poor implementation of the computerized health information management as supposed to be effective in all units, clinics and departments.
