Effective Health Records Monitoring and Evaluation has been demonstrated to be one of the viable tools for proper patient care follow-up. This is affirmed through its use to assess the quality of care given to a patient. This study was therefore aimed at effective health records monitoring and evaluation as ways for proper patient care follow-up. Descriptive research design method was adopted. A total of 282 respondents was randomly selected from the population of study. Yamane Yaro’s formular was used to determine the sample size. A Structured questionnaire was distributed to the 282 selected respondents within the health care institutions. Data obtained were subjected to statistical analysis using simple percentage and chi-square was used to test the hypothesis. Findings from the testing of hypothesis showed that there was significant relationship between health record monitoring and evaluation and proper patient care follow up.
